We only require that you provide a first and last name,  e-mail address on the contact form. Further, if the consultation booking service or chat is available through this site, you may be asked to provide information if you participate in  a consultation booking process or an online chat. Please do not submit any confidential, proprietary or sensitive personally identifiable information (like Social Security Number; date of birth; driver's license number; or credit card, bank account or other financial information) (collectively, “Sensitive Information”). We do not share personal data (phone numbers) and consent with third parties / affiliates or partners. No mobile information will be shared or sold with third parties/affiliates for marketing/promotional purposes. All other categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
